The health care card is obtained by registering with the National Health Service, and compulsory for foreign nationals holding a residence permit for: subordinate /self-employed work, family reasons, asylum, humanitarian reasons, request for asylum, adoption procedures, foster care procedures, citizenship procedures.
In these instances, registration is free of charge. In the case of non-EU foreign nationals holding a residence permit for other reasons, registration is voluntary and involves the payment of a fee.
 
To obtain a health care card, the foreign national must already have a valid Residence Permit. The application must pass the required checks carried out by the competent ASL (Local Health Authority).
 
The applicant must go to the ASL (Local Health Authority) and produce a copy of the current residence permit, or a receipt of application for a first residence permit, or a receipt of application for renewal of an expired permit. The tax code number and self-certification of residence or declaration of current abode are also needed. The ASL (Local Health Authority) health care card is necessary if coming from another Health Authority.
 
In the case of compulsory registration, there is no charge for registration and issue of the card; in the case of voluntary registration, a fee is payable.
 
Issue is immediate.
 
The validity of Registration with the National Health Service is the same as the residence permit. In the case of an application for renewal, registration is valid for 3 months from the date of receipt (renewable).
